Institute for Safe Medication Practices Issues Alert about Possible Fluid Leakage with Parenteral Syringes

The Institute for Safe Medication Practices (ISMP) has issued a National Alert Network (NAN) article that presents information from three hospitals reporting “…occasional instances of medication leaking from syringes.” The NAN Alert states that the issue is occurring when “…liquid is drawn into the syringe rather than after the syringe has been filled.”

The syringes involved in the incidents were manufactured by BD. The company and the U.S. Food and Drug Administration (FDA) Center for Devices and Radiological Health (CDRH) are aware of the situation.
