| JCAHO is proposing revisions to Standard PC.2.120. Currently, Standard PC.2.120 states that hospitals must define, in writing, timeframes for conducting the initial assessment. According to the current standard the history and physical examination (H&P) must be updated at the time of admission if it has been performed within 30 days prior to admission.
