| JCAHO is conducting an anonymous survey about organizations’ techniques and strategies for Patient Self-Management (PSM). According to JCAHO, PSM is the interaction between a care manager (the person who works with patients about their chronic illness such as nurses, educators, physicians, nutritionists, pharmacists, etc.) and a patient. Because the survey is anonymous, any organization that participates will not be identifiable.
