| The JCAHO, in the August 2004 update to the standards manual has added an element of performance under the Management of Human Resources chapter for standard HR.3.20. Element of performance number four (#4) has been added which requires performance evaluations to be documented. The requirements under this additional element of performance became effective July 1, 2004. The wording of the standard and related rationale remains unchanged.
