| The Joint Commission has released a Sentinel Event Alert, Issue 36, April 3, 2006, regarding tubing/catheter misconnections.
According to the Alert, tubing and catheter misconnection errors are an under-reported problem in healthcare organizations. These errors are usually discovered and corrected before any harm befalls the patient.
