| JCAHO issued a Sentinel Event Alert, Issue 35, on January 25, 2006 regarding medication reconciliation to prevent errors. The Joint Commission is urging healthcare organizations to intensify their attention to the accuracy of medications given to patients over the continuum of care. The failure to reconcile medications during transitions of care for patients can cause serious patient injuries and even death.
