| Pressure ulcers continue to be a problem in healthcare today. Assessment and reassessment of residents can help to identify those residents who are at risk for developing pressure ulcers. Risk assessment tools are available to healthcare providers to assess a resident's risk of developing a pressure ulcer such as the Braden Scale and Norton Scale.
