| On May 1, 2006 CMS released revised Medicare enrollment application forms for healthcare providers and suppliers. Under new CMS rules, all providers and suppliers that are not electing to "opt out" of Medicare must complete the standard form, known as a CMS-855 form. The form is to be updated and certified for accuracy every five years.
